  • Overview

    The Family Support Services unit of TVHC provides both intensive case management and outreach to families residing within southern Alameda County (comprised of Hayward, Union City, Newark, Fremont, San Leandro and San Lorenzo). Under the direct supervision of the Senior Director of Population Health the Family Support Services (FSS) Senior Program Manager is primarily responsible for the management, contracts and program activities monitoring, development, implementation and coordination of case management programs, maintaining relationships with funding sources and associated services. The incumbent is accountable for the performance of the case management team, ensuring quality services are being delivered according to standard protocols, quality assurance and productivity measures.

     

    The FSS SR Program Manager’s primary responsibilities are as follows: administration and monitoring of program Memorandums of Understanding (MOUs); supervising staff plan, develop and where appropriate, facilitate FSS department team meetings, serve as a liaison between FSS and the TVHC Executive Management Team; maintain liaison with other departments (Clinical Division, Finance, etc.) as needed and represent FSS at relevant agency meetings. The position requires the ability to effectively lead staff in all areas of program compliance, addressing all performance issues in a timely manner so as to not compromise access to program services. The FSS SR Program Manager will produce regular activity reports to document progress, track client satisfaction / outcomes, and reflect the use of best practices model of therapeutic case management services at TVHC. As a liaison to external stakeholders, the position requires attendance and participation at various program related meetings, trainings, etc.

     

    THE FSS CASE MANAGEMENT PROGRAM

     

    Cal Learn Program:

    The Cal Learn Program is a mandatory program that provides case management services. Teens are referred by the Alameda Social Services Agency. Eligible teens must be under the age of 19, pregnant and/or parenting and without a high school degree or GED and receiving cash aid. Cal Learn offers transportation, parenting support, childcare and needed school supplies. 

    Home Visiting/Family Support Services Program:

    Home Visiting/Family Support Services is a program that is funded by the Alameda County Public Health Department Alameda County Public Health funds (ACPH). The program supports health, development and well-being of young children throughout their first 5 years. More intensive family center services utilizing a multi-disciplined approach are provided in a variety of settings. Families are identified and referred from local hospitals, CBO’s and TVHC staff. These case management services are offered to all Family Support Services clients.

    Targeted Case Management (TCM):

    TCM consists of case management services that assist Medi-Cal-eligible individuals within specified targeted groups to access needed medical, social, educational, and other services. TCM service components include needs assessment, setting needs objectives, individual services planning, service scheduling, crisis assistance planning, and periodic evaluation of service effectiveness. Case management services ensure that the changing needs of the Medi-Cal-eligible person are addressed on an ongoing basis and appropriate choices are provided from the widest array of options for meeting those needs.

    Responsibilities

    As a FSS SR Program Manager for the FSS Case Management Program duties will include:

     

    • Assist Sr. Director of Pop Health with oversight of FSS, case management services at TVHC designated sites; facilitate the integration of TVHC services within the TVHC patient centered medical home (PCMH) model.
    • Provide supervision to FSS Program Manager.
    • Directly supervise case management staff including ongoing monitoring of work quality, conducting regular performance evaluations, developing performance improvement plans, coaching, mentoring and training of new hires in adherence to program policy and procedures.
    • With a focus on meeting overall administrative and program requirements, provide support and assistance to assigned case managers including providing direct case management services to clients in the absence of the case manager.
    • Support the Sr. Director of Pop Health with monitoring the implementation of program objectives and evaluation of services delivered as described in the Scope of Work for each contract.
    • Build, strengthen, and maintain communication and relationship with funding sources and collaborative agencies (MOUs).
    • Work collaboratively with the Sr. Director of Pop Health in the oversight of yearly budget implement and line items costs expenditures.
    • Utilize Lodestar MIS, ECCHange and NextGen to monitor program caseloads, months of service and other indicators, to assess client services productivity.
    • Develop and implement curriculum on client-centered education, provide ongoing staff professional development training as determined by client/staff needs.
    • With input and support from the Sr. Director of Population Health, produce program reports as assigned.
    • In consultation with the Sr. Director of Population Health Director, provide data for annual employee evaluations to assure the adherence to program policies and procedures.
    • In coordination with the FSS Program Manager, perform quality assurance on client files to assure procedures are met and in full compliance with all program contracts.
    • Conduct and monitor the monthly TCM time study and secondary documentation in collaboration with the FSS Program Manager.
    • Co-facilitate the monthly program staff meeting and case conferences along with the FSS Program Manager.
    • Support the Program Administrator with monitoring and maintaining internal and external program referrals and ensure case managers maintain a case standard as it relates to contract compliance.
    • In coordination with the Senior Director of Population Health Director, maintain a network of service providers and resources for Family Support Services in Southern Alameda County.
    • Maintain collaborative relationships with school districts, local schools, to monitor the continued flow of new program referrals.
    • Participate in program trainings as directed by the Senior Director of Population Heath.
    • Participate as a member of the Population Health management team to ensure all staff is covered clinically as needed when other clinical managers are on vacation, or out sick as appropriate.
    • Maintain in good standing with the Board of Behavioral Sciences in order to provide clinical supervision to licensed eligible staff.
    • Develop and Implement Staff retreats with FSS Program Manager as directed by the Senior Director of Population Health.
    • Work in conjunction with the Senior Director of Population Health, the FSS Program Manager, and the Business Development department in identifying and applying for grants for program sustainability.
    • In collaboration with FSS Program Manager make modifications and/or additions to the FSS Standards and Procedures Manual, as needed.
    • Maintain confidentiality regarding privileged administrative and client information in a professional manner; ensure all levels of service delivery adhere to HIPAA and clinical confidentiality standards.
    • Maintain a network of service providers and resources for Family Support Services in Southern Alameda County.
    • Attend all appropriate clinic meetings to coordinate case management and support services with other TVHC services and sites.
    • Those duties as assigned by the Sr. Director of Population Health.
    • Able to meet or exceed the Service Excellence Standards of TVHC, Inc.

     

    Qualifications

     

    • Knowledge of health information related to parenting and pregnancy.
    • Maintain networking relationship with community service providers.
    • Have valid California Drivers Licenses, auto insurance and accept travel assignments on the job as directed by the program.
    • Working knowledge of QA standards and procedures.
    • Experience in administration, coordination, program development, supervision and case management.
    • Previous experience working with adolescents and high risk populations, including sound crisis management skills.
    • Working knowledge of typical/atypical child development, the parent- child relationship and attachment.
    • Strong ability to work as a team member with fellow co-workers, supervisor and agency staff.
    • Cultural sensitivity and understanding of special family and youth issues.
    • Ability to perform program outreach.
    • Demonstrated flexibility in work schedule, job duties and assignments.

     

     

    Education and Experience:

    • Master’s Degree in Social Work, Psychology, and Counseling or related field or equivalent preferred.
    • Licensed Clinical Social Worker (LCSW) or Licensed Marriage, Family, Therapist (LMFT) with at least 1-2 years working as LCSW or MFT strongly preferred.
    • 1-2 years minimum experience in administration and supervising professional level employees, with evidence of progress in successfully assuming increased management and planning responsibilities.
    • Working knowledge and experience in healthcare program development and implementation experience.
    • Experience with implementing start-up projects successfully.
    • Previous experience working with culturally diverse adult populations.
