Under the supervision of the Integrated BH Clinical Supervisor, the BH Services Care Coordinator is an essential member of the primary care - behavioral health care team. The position is responsible for promoting improved access and care management for clinic patients who have medical and behavioral health care needs. This position includes being a single point of contact to receive warm hand offs from external behavioral health care agencies and facilitates care transitions between the specialty mental health system and the primary care medical/health home. This position also promotes timely and coordinated internal communications among clinic primary care providers, behavioral health specialist, consulting/treating psychiatrists and patients. In order to maximize patient access and provider productivity, this position also collaborates with clinic staff, manages provider schedules and client appointments, follows up with no shows, facilitates warm hand offs between medical and behavioral health care staff, and provides referrals for community resources to support whole person health. This position utilizes the electronic health care record and population management tools, to collect data, communicate with providers and produce reports to support collaborative care, improved health outcomes and program compliance. The positions also produces and is responsible for weekly panel management reporting and distribution, and other key reporting for the Integrated BH Programs at TVHC.


The Healthcare Coordinator - BH Services Care Coordinator has a lead role in the design, implementation and monitoring of clinical care programs to ensure the highest utilization for optimal health outcomes in affected patient groups. This may include but is not limited to planning of clinical protocols related to BH, Chronic Care, and other health conditions, internal promotion through health education and leading of discussion groups among BH and Primary Care providers Clinical Staff. From a data analyst approach the position also works closely with IT, EHR, and other data programs to facilitate the flow of care for patients while at the same time capturing critical data programs for reporting purposes. The Healthcare Coordinator must also aggregate data according to QA and CQI initiatives, drafting reports and producing publish-ready documentation for internal and external stakeholders.


  • Accepts warm hand offs between agencies and coordinates care for patients to facilitate integration of care
  • Accepts warm hand offs of patients between medical and behavioral health providers to facilitate integration of care
  • Assists in establishing and maintaining smooth functioning and relationships between primary care providers, behavioral health specialists, and primary care consulting psychiatrists.
  • Tracks, monitors and reports client attendance and follows up on no-shows to support and improve continuity of treatment
  • Tracks, monitors, and reports depression and/or anxiety screenings within primary care and behavioral health and follows up as directed with patients and/or in assignment of patients for follow up by behavioral health specialist
  • Be fully knowledgeable of patient eligibility requirements for additional service and assist patients in navigating their care with both in-house and outside providers by facilitating scheduling of visits, identifying barriers to receiving care, and assisting with providing referrals to community and internal resources
  • Participates in referrals under the supervision of the BH Clinical Supervisor for patients who are experiencing a crisis or require a higher level of care to facilitate the referral and to facilitate and confirm contact with service provider, if further services are needed
  • Provide health education and patient instructions as defined by protocol and as appropriate to promote the understanding and adherence to BH screening and treatment as well as in coordination with other appropriate health and wellness education
  • Provide sensitive and accurate information to patients, including those being screened and treated for BH conditions, including those patients needing referral to outside providers
  • Participates in, coordinates, and/or attends and conducts outreach at community events such as health fairs and community forums, as a representative of TVHC, Inc.
  • Maintains updated health information as directed through direct inquiry or database reconciliation using a variety of platforms including EHR (NextGen), i2i Tracks


  • Associates degree
  • Experience in health, social services or a mental health setting preferably with knowledge of Bay Area health and social service resources
  • Strongly prefer outreach, education, and in medically under served communities and in a community health center setting
  • Bilingual in Spanish and English is required
