Tiburcio Vasquez Health Center (TVHC) is a leading nonprofit community health center that has provided comprehensive medical, dental, and mental health care since 1971 that primarily serves the community's underserved population.  TVHC is located in Hayward, California and is part of the beautiful San Francisco East Bay.


This Monday-Friday/Full-time position, reporting to Executive Vice-President/Chief Administrative Officer (VP/CAO), will primarily be responsible for assisting the VP/CAO in the implementation of a strategic vision to drive performance improvement in the areas of employee engagement and training, organizational quality and communications. Leads critical decision making that drives change transformation and assists in the strategy execution to support the overall vision of TVHC. Has direct involvement in project management and performance measurement of corporate projects.


This individual will initiate and promote a culture of continuous improvement by connecting, rationalizing and maturing all of our change and improvement efforts into an effective and cohesive approach in support of our vision and strategy. This role will develop an integrated practice of overseeing organizational-wide programs/projects and will guide a portfolio of strategic initiatives, as well as further develop our collective skills, knowledge and tools.


Helps to facilitate organizational excellence strategies through operational efficiencies and productivity across multiple department and functions. Leads process gap analyses and drives major change management efforts to promote a culture and mindset of operational excellence and productivity.

Responsible for developing, initiating, leading and driving continuous improvement projects. This includes process or business changes to improve staff retention and education, staff development, quality assurance, and risk management.

  • Assists with development of TVHC’s organization-wide key strategic plans and manages the oversight of their progress.  Drives objective-setting and execution of the projects with the objective to optimize efficiencies and use of resources.
  • Requests project team members from different functions, and is accountable for creating, forming, and leading brand new cross-functional project teams to solve complex problems.
  • Oversees individual and/or group training and instructional programs, using instructor-led, web-based and blended learning courses, self-paced or self-guided instructional guides, and/or multimedia training methods and formats.
  • Evaluates learning effectiveness and implements program improvements to ensure they reflect industry best practices and meets organizational needs.
  • Collaborates with HR and Quality and other business partners, subject matter experts and management to assess needs and identify performance development solutions.
  • Conducts testing and pilots performance improvement solutions and other processes necessary to evaluate the effectiveness of training courses, materials, and job aids.
  • Responsible for successful project implementation by defining effective performance objectives and the implementation of organizational-wide business plans.
  • Works closely with VP/CAO to identify, define and develop key metrics and management processes that drive organizational performance across TVHC.
  • Develops and reports performance excellence progress and metrics. Monitor report and assure transparency of performance throughout organization.
  • Assists with setting standard operating procedures and best practices; identifying key performance indicators; targeting opportunities for improvement; analyzing data and conducting improvement initiatives.
  • Develops and implements departmental or workgroup intervention strategies or best practices to improve performance through work group facilitation, team building, mediation, professional skill building, or organizational change initiatives.
  • Advises and coaches management and supervisors on implementing performance improvement strategies.
  • Consults with managers and supervisors to devlopo employee performance improvement measures and may assist in devloping individual plans to improve employees' performance.
  • Provides leadership by mentoring associates in the use of Lean tools, ensuring transfer of knowledge and creating a culture of continuous improvement.
  • Facilitates the preparation of Annual Action Plans and manages the oversight, through completion, of such plans in support of long range strategic plans.
  • Reviews results from annual Customer/Staff Satisfaction Surveys within areas of program responsibility, aggregating data, analyzing survey trends and working collaboratively with program staff regarding follow-up activities and improvements, as required.
  • Assists with the formulation, implementation and monitoring of corrective actions to ensure program improvement including but not limited to: policy/procedure development and review, systems development and review, establishing benchmarks, personnel assignments, training, etc., tracking the implementation of corrective actions providing follow-up, as needed.
  • Identifies and facilitates process improvement projects, training/workshops to drive cultural change.
  • Carries out advanced data collection and analysis for process mapping, develop budgets and cost analysis to determine project feasibility.
  • Responsible for specific deliverables, outcome indicators, and performance-measurement indicators for assigned projects.


Required Education and Experience:

  • Bachelor's degree in Education, Communication, Business, Organizational Development, Public Health, or related degree; or equivalent.
  • 3+ years of change management experience (this can include; process, organization, communication, and/or learning improvement experience).

  • Proven experience in developing and implementing training programs; proficient in organization development; ability to manage multiple projects in a consultative, efficient and effective manner in a highly collaborative and diverse environment.Ability to identify potential people-side risks and anticipated points of resistance, and develop specific plans to mitigate or address concerns.
